1. Completed registration forms, if they were sent to you in advance.

2. X-rays, CT Scans, MRIs or any other diagnostic tests relating to your condition.
   
   
3. All insurance information, including insurance cards, insurance forms and referral authorization if necessary.

1. Payment is expected at the time of service unless prior arrangements have been made.
   
   
2. If you have specific questions you would like to ask your physician, it may be helpful to write them down and give them to your doctor during your office visit.
   
   
3. If you require a note or completed form from the doctor for work or school, please discuss your need with our staff before you leave the office.
